dc.description.abstractAdherence to antiretroviral therapy (ART) is critical for the effective management of HIV/AIDS. A minimum adherence rate of 95% is necessary to achieve optimal health outcomes, yet many patients struggle to maintain this level. This study investigates the factors influencing non-adherence among adults living with HIV/AIDS at Kisumu County Hospital, Kenya. Methods: A cross-sectional study was conducted involving 150 adult clients aged 18 and above who have been on ART for at least three months. Participants were selected through systematic random sampling from a total of 234 active ART patients at the Comprehensive Care Clinic (CCC). Data were collected using structured questionnaires and semi-structured interviews, incorporating both qualitative and quantitative methods. The data were analyzed using SPSS software version 22, with results presented in charts and frequency tables. Results: The findings revealed that 60% of participants reported good adherence, missing no more than one dose per month. However, 40% of respondents identified various barriers to adherence, including side effects (40%), distance to the clinic (15% living more than 10 km away), and financial constraints (20% reporting significant cost impacts). Notably, 80% of respondents felt supported by healthcare providers, which positively influenced their adherence. Conclusion: This study highlights that sociodemographic factor, accessibility to services, and perceptions of treatment significantly influence ART adherence among adults living with HIV/AIDS. Enhanced support systems, targeted educational interventions, and strategies to alleviate financial burdens are essential for improving adherence rates. Future research should explore longitudinal adherence patterns and the efficacy of specific intervention strategies.en_US
