dc.description.abstract | Medication administration errors(MAE’S) are among the most prevalent medication errors worldwide. Underlying system factors have been seen to be crucial contributors to medication errors. By understanding the causes of these errors, the most appropriate intervention can be designed and implemented to minimize the occurrence. Health care in Kenya have been susceptible to occurrence of medication errors, some of which may lead to permanent disability and loss of life of the patient. The medical error reporting system is a method of identifying medical errors and reporting them to the relevant authority, though there is underreporting. This study aims to systematically review and appraise empirical evidence relating to causes of medication administration errors in hospital settings. The study aimed at examining factors contributing to medication errors at Kisumu County Referral hospital medical wards. The study design was across sectional study design. The study population consisted of medical officers, clinicians, and nurses. Purposive sampling technique were used. A total of 30 staffs were sampled for self-administered questionnaires with a response rate of 95%, confidentiality ensured through all process of data collection. The survey questionnaires responses were analyzed using Statistical package for social science (SPSS), the findings were presented using graphs, charts, and tables. The study found out that Organization factors, providers' factors and patient factors contributed much to the occurrence of errors in the medical wards. Patient factors contributed much to underreporting of the errors encountered; Lack of feedback after reporting the errors, fear of colleagues expressing negative attitude to a medical practitioner who committed an error, Fear of impact of reporting errors on staffs' annual appraisal and culture of blaming individual instead of system were among the patients' factors contributing to underreporting. More Research should be conducted to assess the major causes of medication errors and underreporting of the errors in KCRH. Data from various research conducted on the various factors contributing to medication errors both in public and private hospitals should be assessed and analyzed in order to come up with various ways of preventing it thus improving patient safety in Kenya. | en_US |
